Proof of Life serves individuals who have experienced interpersonal violence and who require restorative medical care for the physical reminders of that violence: scars, disfigurement, burns, dental damage, and other visible or functional consequences of injury.
We serve adults and emancipated minors in the United States. We do not require a police report, a court order, or participation in any program as a condition of receiving a grant. We do require medical documentation of the injury (from any licensed provider) and a specialist referral or medical necessity rationale.

Plastic and reconstructive surgery for visible deformity, structural injury, or functional impairment resulting from violence. Includes soft tissue reconstruction, bone reconstruction, and revision of prior surgical repair.
Restorative dentistry and oral/maxillofacial surgery for dental damage caused by violence. Implants, reconstruction, orthodontic repair, and jaw reconstruction as medically indicated.

Unlike traditional grant-making models that disburse funds to applicants directly, Proof of Life grants are paid to network providers at their direct operational cost.
Here is what that means in practice.
When your grant is approved, you choose a provider from our vetted network. We communicate directly with that provider to schedule your care. The foundation pays the provider at direct cost — no service fees, no profit margin — for the approved procedure. You pay nothing, and you are never invoiced.
This structure exists for a reason. It is the most efficient conversion of donor capital into care. And it removes the burden of navigating a financial transaction with a medical provider during an already difficult time.

Complete our confidential online application. You will describe the injury, the restorative care being sought, and provide one or more verification pathways. The form takes approximately 20 minutes.
We request medical records or a specialist referral documenting the injury and the medical rationale for the requested care. We work with applicants to identify the most accessible documentation pathway — this can come from any licensed provider who has treated you.
Our medical advisory committee reviews each application against clinical criteria. The committee evaluates procedural appropriateness, specialist match, and expected outcome. The committee does not evaluate character or narrative.
Approved applicants receive a grant offer and a list of network providers qualified to deliver the approved care. You choose the provider. We do not route you to a specific clinic.
The foundation coordinates directly with your chosen provider. They deliver the approved care. We pay them at direct cost. You receive the care, and no bill ever reaches you.
Thirty and ninety days post-care, we conduct a brief, voluntary outcome check-in. This is not surveillance. It is how we measure grant effectiveness and improve the criteria we use for future applications.

All applications are treated as confidential. Personal information is never shared with law enforcement, immigration authorities, or any third party without explicit written consent, except as required by law in cases involving imminent danger to a minor.
Individual grants are sized to the medically-indicated procedure, paid at network provider direct cost. Typical grants range from $2,000 to $30,000 depending on the category and clinical scope.
In rare cases where no network provider can deliver the medically-indicated care, the committee may authorize an out-of-network exception. These are evaluated case by case.
Proof of Life funds restorative medical care through our network. We do not provide emergency financial assistance, housing support, legal representation, or cash grants. We maintain a referral network for complementary needs.
Board-certified specialists in reconstructive dermatology, plastic surgery, maxillofacial surgery, restorative dentistry, and trauma-adjacent procedures who are interested in delivering care to grant recipients at direct operational cost should contact the foundation.
Network providers are nominated by peer referral, reviewed by the medical advisory committee for clinical credentials, and re-certified annually. Provider-aligned philanthropy is the economic foundation of our model — and its most distinguishing feature.
